I have appended the form; it was adapted from the original AW form. It's purpose was to allow us to print a proceedings and give it to future editors or other interested people. Since the proceedings have a Siemens TR number we thought we need this kind of protection.

I don't think you need it for PLoP if you provide only Kinko proceedings (as opposed to UIUC TR or something like that).

If the publisher web-site solution is reaching more concrete forms, it might be sensible to adapt this permission to include "publication" on such a web-site. -- DirkRiehle

I grant permission to print the paper entitled

as part of a Siemens Technical Report and to distribute it at EuroPLoP '97 and to interested third parties.

The copyright statement on the first page of my paper shall be:

"Copyright 1997 by XXX"

I will retain the copyright on the paper itself.

I assert that I control these rights in their entirety.
